PrescribeWellness Can Now Further Support Care Coordination and Other Enhanced Services with North Carolina Pharmacies as the First Level 3 - Active Medications-Capable Clinical Documentation Platform

PrescribeWellness, the cloud-based services company whose mission is to inspire collaboration for better health, today announced that it has achieved Level 3 - Active Medications capability from Community Care of North Carolina (CCNC) for the Pharmacist eCare Plan initiative. Using PrescribeWellness' PrescribeCare solution, community pharmacies across the country can share care plans electronically and support care coordination. Pharmacies in North Carolina are actively participating with CCNC and its patient-centered medical home and care management activities using PrescribeCare. Because they can submit the eCare Plan from their clinical documentation system, they are also now eligible for per member, per month payments for longitudinal care and follow-up. PrescribeWellness is the first technology provider to achieve Level 3 - Active Medications capability, which converts the Fast Healthcare Interoperability Resources (FHIR) standard to Consolidated Clinical Document Architecture (C-CDA) and vice versa, to communicate patient active medication encounters—such as medication therapy problems, medication action plans and recommendations—to therapeutic goals and patient medication lists.

PrescribeCare: Driving Enhanced Patient Services

PrescribeCare is an electronic pharmacy "care plan" that pharmacists can use to electronically communicate with other care providers, reducing redundant data entry, improving care coordination, and earning community pharmacies compensation for their care interventions.

Community pharmacists can also use the PrescribeWellness technology platform to identify what interventions or services a patient needs and drive patient services like medication reconciliation, adherence monitoring and medication synchronization, immunizations, Medicare plan reviews, patient engagement, communication to the electronic health record to other healthcare providers, and billing for enhanced services.

Level 3 capabilities like sharing active medication lists break through the traditional construct and limitations of conventional pharmacy HIT systems in that they are the building blocks of the pharmacy clinical record and transcend dispensing," said Troy Trygstad, vice president of Pharmacy Programs at Community Care of North Carolina. "Active medications can be shared and understood regardless of who filled them or if they were ever filled at all. As technology partners such as PrescribeWellness now begin to work on other elements such as lab values, vital signs and time tracking of effort, pharmacists can become valuable members of the care team and develop the requisite capabilities to participate in payment reform efforts."

"PrescribeCare is very user-friendly—it is very easy to look over meds and document side effects," said Dustin Webster, pharmacist at South Park Family Pharmacy-Winston Salem, who has submitted over 230 eCare Plans to CCNC. "With the possibility for provider status, PrescribeCare allows us to show our value by making it easier to document our interventions and address patient non-adherence, so we are able to keep more patients out of the hospital."

PrescribeWellness interfaces with over 50 pharmacy management systems to analyze dispensing data in real time, giving the pharmacy team insight into the needs of their patients and streamlining the medication reconciliation process. Community pharmacies that are not current customers of PrescribeWellness can purchase PrescribeCare as a standalone platform. For current PrescribeWellness customers, PrescribeCare is available for no additional cost.

Supporting the Work of Community Care of North Carolina (CCNC)

PrescribeWellness developed PrescribeCare to support CCNC, a network of more than 6,000 primary care providers that provide targeted services to an eligible population of more than three million Medicaid, Medicare and dually-eligible recipients. It also supports Community Pharmacy Enhanced Services Network (CPESN) organizations in their care coordination and patient care initiatives and the CCNC CMS Centers for Medicare and Medicaid Innovation project.
