Health Care Transformation Task Force Grows, Adds Two Members

The Premier Alliance of Private Sector Leaders Spearheading Adoption of Value-Based Payment Models Is Growing Its Providers Section

WASHINGTON, Aug. 7, 2017 /PRNewswire/ -- Apollo Medical Holdings, Inc. ("ApolloMed") AMEH.  The Health Care Transformation Task Force (HCTTF), a consortium of leading health care payers, providers, purchasers and patient organizations working to accelerate the pace of U.S. health care delivery system transformation, announced today that ApolloMed and ConcertoHealth have joined its membership.

"ApolloMed and Concerto Health are leaders in the transformation to value-based care, and their knowledge and commitment to this important transition will be invaluable as we continue to move the health care sector to better serve patients," said Jeff Micklos, Executive Director of the Health Care Transformation Task Force.

With more than 40 members representing patients, payers, providers and purchasers, as well as academic and policy leaders, the HCTTF is further aligning private and public sector efforts to transform the U.S. health care system. With deep experience in both private and public sector health care, the Task Force is uniquely positioned to develop and disseminate strategy, operational and policy recommendations to spark rapid, measurable change across the health care landscape.

"We believe that HCTTF's mandate aligns very well with ConcertoHealth's enduring commitment to delivery system design and healthcare policy that promote patient-centered care, particularly for the benefit of frail, elderly, and high risk patient populations across the country," said Colin LeClair, Chief Development Officer for Concerto Health.

Already operating exclusively in value-based agreements, ConcertoHealth provides high-touch, individualized care for patients, and also deploys wraparound clinical resources to extend the reach of primary care practices.

ApolloMed is a leading integrated healthcare company providing solutions to hospitals, health plans, physicians, and other health providers to provide cost-effective, quality healthcare.

"Our organization is and has been a strong advocate of the goals of the triple aim as well as the transition to value-based payment models," said Warren Hosseinion, Chief Executive officer and Co-Founder of ApolloMed. "We are thrilled to be joining Health Care Transformation Task Force to help further our goal of delivering high-quality care to patients across the entire care continuum."

As part of the Health Care Transformation Task Force, members are committed to put 75 percent of their business into value-based arrangements that focus on the Triple Aim of better health, better care and lower costs by 2020.
