, one of the country's leading health and well-being companies, announced a new Accountable Care agreement focused on promoting evidenced-based, high-quality care that can lead to improved medical outcomes, a better customer experience and lower costs. Humana's newest Accountable Care relationship took effect Oct. 1, 2014, and is designed to serve the needs of Humana Medicare Advantage members based in Hawaii. “MDX Hawai‘i has been an administrator of health plans in Hawaii for over 30 years. Our vision for this collaboration with Humana is to bring outstanding local service to Humana Medicare Advantage members,” said Rick Jackson, Chief Operating Officer at MDX Hawai‘i. “Kina‘ole, as introduced to us by the late George Kanahele, has been part of our culture that has made us successful over the years. This philosophy of ‘doing the right thing in the right way, the first time' drives us to provide the health services in a manner that brings dignity and respect to our Medicare Advantage members and large network of excellent providers.” MDX Hawai‘i Preferred Provider Network includes more than 3,700 qualified providers of every major specialty and hospitals throughout the State of Hawaii. “This Accountable Care partnership with MDX Hawai‘i is a natural fit for Humana as we already share a focus on integrated health and patient-centered care,” said Mike Franks, Market Leader of Humana's Senior Products in Hawaii. “We believe our Medicare Advantage members in Hawaii will greatly benefit from this agreement, helping them maintain their health and well-being.” The new Accountable Care agreement between Humana and MDX Hawai‘i includes value-based incentives tied to performance improvement and will bring better patient care coordination, more cost effective, quality care and a world-class patient experience to Humana members in Hawaii. The arrangement will offer more personalized care and a broad range of population management tools like, predictive analytics and chronic care, disease management and wellness programs. The value-based arrangement also emphasizes quality outcomes with a focus on care coordination and leverages measures as defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) like; diabetes care and treatment, breast cancer screenings, colorectal cancer screenings and high-risk medication. Humana has a 25-year Accountable Care relationship history with more than one million members that are cared for by 33,000 primary care physicians, in more than 900 Accountable Care relationships across 40 states and Puerto Rico.
