To help drive down patient readmissions and enhance care, Northwell Health will integrate predictive artificial intelligence (AI) software from Jvion's Cognitive Machine into the health system's electronic medical records (EMR) at 15 hospitals.

Georgia-based Jvion and its AI solution will be used to identify at-risk patients and other metrics that contribute to an individual's increased risk. This includes social determinants such as a lack of access to transportation as well as food and housing insecurity. During the initial phase of implementation, Northwell Health will target readmissions and avoidable admissions within its managed care population.

"Advanced technology is a critical part of our commitment to creating a health system that is the highest value in the country," said Kristofer Smith, MD, Senior Vice President of Population Health Management at Northwell Health Solutions. "By incorporating the proven AI delivered by Jvion's Cognitive Machine, we will extend our ability to deliver quality, safe, and cost-effective care."

The goal is to seamlessly integrate the Cognitive Machine's risk and recommendation outputs so that caregivers have access to the information within the existing clinical workflow. Jvion's solution will proactively manage patients by flagging those at-risk while providing the clinical drivers and other factors that contribute to the likelihood a readmission or avoidable admission. It will also deliver individualized recommended actions to best mitigate risk on an individual basis.

"Northwell Health is taking the lead in preventing avoidable events and ensuring that care is delivered in the most effective way," said Dr. John Showalter, Chief Product Officer for Jvion. "Preventing a deterioration is complicated and dynamic. Jvion's Cognitive Machine has made significant impacts to various readmission and avoidable admission programs across the country and we are very proud to bring it to Northwell Health. Using the power of cognitive machine technology, Northwell Health will be better able to improve patient outcomes and deliver higher quality, more efficient care."
