Malvern, PA and Washington, D.C. (PRWEB) August 07, 2018

HMP, a leading healthcare education and events company, and the National Committee for Quality Assurance (NCQA), a not-for-profit organization dedicated to improving healthcare quality, today announced the 4th Annual Patient-Centered Medical Home (PCMH) Congress will take place September 14-16, 2018 at the San Diego Convention Center in San Diego, California.

The Patient-Centered Medical Home is a model of care that puts patients at the forefront of care. Research indicates the PCMH model improves care quality, the patient experience, and staff satisfaction while reducing health care costs. NCQA's Patient-Centered Medical Home Recognition Program is the most widely adopted Patient-Centered Medical Home evaluation program in the country. More than 12,000 practices (with more than 60,000 clinicians) are recognized by NCQA.

PCMH Congress is the annual educational gathering for recognized practices, those who are considering recognition, and others interested in patient-centered care. The 3-day meeting attracts nearly 1,000 attendees who will receive tips, tools, and ideas for immediate application through interactive conference sessions tailored to their organization's needs; engage one-on-one with leading PCMH experts who can assist in answering critical questions; and network with peers from around the country who can share insight into common challenges and offer potential solutions.

The 2018 PCMH Steering Committee includes:

  • Michael S. Barr, MD, MBA, MACP, Executive Vice President of Quality Measurement and Research Group, NCQA
  • Patricia Marine Barrett, MHSA, PCMH CCE, Vice President of Product Design and Support, NCQA, Washington, DC;
  • Candace J. Chitty, RN, MBA, CPHQ, PCMH CCE, President, Quality First Healthcare Consulting Inc;

"We're very excited about this year's Patient-Centered Medical Home Congress," said Tricia Barrett, Vice President of Product Design and Support, NCQA. "This year's Steering Committee has designed a robust, high-quality program with tracks focused on behavioral health, collaborative care, health IT, payment models, and PCMH transformation. Participants will have the opportunity to learn not only from experts, but also from one another, as they gather as a community to improve care delivery and collaborate as members of the ‘Medical Neighborhood'."

Delivering the keynote address on how teamwork impacts patient care is Greg Bell, Founder of Water the Bamboo Center for Leadership. An innovator and keen observer of highly successful leaders and teams, Bell will provide an "under-the-hood" look at the characteristics of high-performing teams and explore how to build and nurture teams that consistently perform better, innovate more, and achieve collaborative success.

"Our goal is to facilitate best practices and practical solutions that attendees can take back to their organizations and implement for immediate improvements to patient care," said Randy Robbin, President of NACCME, the medical education subsidiary of HMP. "I believe this year's program and particularly, our keynote helped achieve that goal. Greg's highly engaging storytelling, and his ability to masterfully blend insights and encouragement will make for an impactful address to our attendees on the power and potential of effective teamwork."

Also featured on the 2018 agenda is Paul Grundy, MD, MPH, FACOEM, FACPM, the "Godfather" of the Patient-Centered Medical Home revolution. Dr. Grundy is a recipient of the 2016 Barbara Starfield Primary Care Leadership Award and the 2012 NCQA Quality Award. Dr. Grundy will share valuable insights on the changing healthcare landscape and introduce some of the organizations who are driving this change toward a more patient-centered approach.

Participants in the 2018 PCMH Congress can earn up to 21 CE credits for physicians, pharmacists, and nurses or 15 MOCs for PCMH Certified Content Experts.
