ZeOmega Wins CMO, Montefiore Care Management Business with Jiva Population Healthcare Management Platform

ZeOmega Inc., the premier provider of software solutions for population healthcare management, today announced that CMO, Montefiore Care Management, a unit of Montefiore Medical Center in Bronx, N.Y., has selected its Jiva™ care management software to enhance coordination and improve patient engagement. ZeOmega's solution components, including care analytics, care plans, dashboards, assessments and portals, will be integrated into the CMO's care management programs.
