CAC-Florida Medical Centers, a subsidiary of health and well-being company Humana Inc. HUM, has achieved certification by the National Committee for Quality Assurance (NCQA) as a level three Physician Practice Connections® – Patient-Centered Medical Home™, recognizing the company's long-time commitment to high quality primary and preventive patient care at its full-service medical centers in Central and South Florida.

The Patient-Centered Medical Home emphasizes access, communication and patient involvement, and has proven successful at improving the quality of primary care, while lowering costs and increasing patient satisfaction.

At the helm of the medical home model are primary care doctors who demonstrate they coordinate all patient care with a team of health care providers, who systematically track tests and results to better identify patients at risk, and who provide prompt access to care and follow up after visits.

The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians, American Academy of Family Physicians, American Academy of Pediatrics and American Osteopathic Association and others.
