CareMore Nevada Selects Allscripts for Chronic Care Management (CCM) Program

Solution enables the improvement of preventative maintenance and long-term care planning for patients with chronic conditions

CHICAGO, May 5, 2015 (GLOBE NEWSWIRE) -- CareMore Medical Group of Nevada (CareMore) in Las Vegas, Nev., selected Allscripts MDRX Chronic Care Management (CCM) program to assist in rapidly improving patient engagement and quality of care while increasing Centers for Medicare & Medicaid Services (CMS) reimbursements.

CareMore is an existing Allscripts Professional EHR™ client using Allscripts hosted solutions which wanted to supplement existing resources with a turnkey CCM solution to improve patient outcomes while limiting impact on existing workflows and operations. With Value-based Care Services from Allscripts Professional Physicians Business, the Allscripts CCM program can offer the flexibility of a fully scalable program.

With a well-established Medicare patient base, the practice also wanted to increase fee-for-service and reimbursement revenue within their existing patient volume. CareMore selected the Allscripts CCM program to enable the practice to leverage annual wellness visits and initial preventative physical exams for long-term health maintenance.
