Qualcomm Announces New Connected Health Collaborations at Connect 2014

, King's Daughters Medical Center and CareCentrix to develop next-generation connected health solutions powered by Qualcomm Life's transformational 2net™ and HealthyCircles™ platforms, enabling continuous care, informed interventions and better management of at-risk populations. The announcement was part of Qualcomm Life's annual Connect 2014 conference taking place on August 11-12, 2014 in San Diego, California. By integrating with Qualcomm Life's scalable solutions, these companies now have access to one of the world's largest open and interoperable mHealth ecosystems of medical device, applications and service companies in the health care industry. By liberating vital health and biometric data, Qualcomm Life is assisting these companies with the clinical context and exception-based management tools to efficiently enable care team coordination for transitional care and chronic care management. ResMed, a leader in innovative medical equipment for sleep-disordered breathing and other respiratory disorders, is using Qualcomm Life's technology to streamline transitions of care from the hospital and into the home. Of note, ResMed is integrating the 2net Hub and Platform with its Astral™ 100 and Astral 150 life support ventilators to seamlessly capture and transfer near real-time biometric data to the HealthyCircles care coordination platform, which allows care teams to effectively monitor and manage patients by exception. Additionally, ResMed is using HealthyCircles in combination with its continuous positive airway pressure (CPAP) device to enable hospitals and care providers to closely monitor patients to prevent hospital readmissions. "Working with Qualcomm Life extends our data solutions into the next-generation of remote patient monitoring solutions," said Jeremy Malecha, senior director, Product Management, ResMed-Americas. "The 2net and HealthyCircles Platforms infrastructure streamlines data collection and transmission, enabling continuous, quality respiratory care for patients beyond the hospital." King's Daughters Medical Center (KDMC), located in Ashland, Ky., is collaborating with Qualcomm Life to deploy and scale its health services to manage employers' high-risk patient populations with complex care needs in Kentucky, Ohio and West Virginia. Focused on patients with heart failure, chronic obstructive pulmonary disease (COPD) and asthma, KDMC's integrated care coordination and management services will increase touch points in the rural community and link community-based providers through the 2net and HealthyCircles Platforms. "Qualcomm Life's strong leadership in the connected health space makes them the ideal collaborator for our preventative, transitional and complex care services," said Matt Ebaugh, Chief Strategy Officer at KDMC. "The 2net and HealthyCircles Platforms will allow us to develop reliable and cost-effective complex care coordination and management solutions to closely monitor and care for our high-risk patients in rural communities." David Ziegler, Director of Mobile Product Management at WebMD, will be speaking about the company's efforts to make health information actionable. Ziegler will provide an overview of Healthy Target, a behavior change program within its flagship mobile app, which leverages Qualcomm Life's 2net Platform to aggregate biometric data with its rich set of content and tools to empower consumers to better manage their health.
